You need to know where you have been to know where you are going with your weight loss program.

 

Dieters need a roadmap. Using scales, measuring yourself, and graphing your progress on a chart all provide visual stimulus and a reference to keep you going.
 

Medical Records Release Form

  1. Download and Print form
  2. Complete form.  Form must be signed and dated.
  3. Fax to 405-330-2363
  4. or Mail to:  200 N Bryant Suite 100, Edmond, OK 73034

Please allow 14 business days from receipt of completed medical records release for request to be processed.  The Medical records custodian is onsite two days a week.  A charge may apply to some requests.  You will be notified with the amount prior to your request being completed if a charge applies.
