Problems Downloading or Printing?

If you experience problems downloading or printing these forms please contact the office at 405-340-2362 (press option 0) between 8am and 5pm.  If you experince problems after 5pm the day prior to your appointment please arrive 1 hour prior to your appointment to complete parperwork.  For appointments before 9am please arrive at 7:45 to complete paperwork.

Medical Records Release Form

  1. Download and Print form
  2. Complete form.  Form must be signed and dated.
  3. Fax to 405-330-2363
  4. or Mail to:  200 N Bryant Suite 100, Edmond, OK 73034

Please allow 14 business days from receipt of completed medical records release for request to be processed.  The Medical records custodian is onsite two days a week.  A charge may apply to some requests.  You will be notified with the amount prior to your request being completed if a charge applies.
